Application Instructions
- Print the transcript request forms below. By submitting these completed forms, Midway College is able to request transcripts on your behalf. Complete a transcript request form for each prior college you have attended. If you have earned less than 24 semester hours, please also fill out a form for your high school transcript.
- Complete this online application. When you are finished, click the 'SUBMIT' button on the left column and follow the instructions.
- Print the application agreement statement that appears after you submit the application. Sign and mail it — with the required nonrefundable $25 application fee and the transcript request forms — to SCD Admissions at the address below.
Financial Aid and Scholarships
- A student must complete the Free Application for Federal Student Aid (FAFSA) to be considered for federal financial aid. Forms are available after January 1 and should be completed by April 1 in order to receive maximum financial aid. You may apply online through FAFSA on the Web or obtain the forms from your high school guidance counselor or from the Midway College-SCD, Office of Admissions.

The requested information is used to determine eligibility for admission; to evaluate academic history; to support financial aid requests; to conduct demographic research for program evaluation; to comply with state and federal reporting requirements; and for other uses as required by law or accreditation standards.

How to Use the Midway College School For Career Development Online Application
|
To complete the online application, simply type your answers in the corresponding fields or select your answers from
the popup lists (where applicable).
After you complete a screen, click the NEXT button at the bottom or on one of the sections listed on the left
column to move to a different screen. Before you are allowed to jump screens, we will inspect your data on the current
screen for errors or inconsistencies. If errors or omissions are found, you will be returned to the screen to correct
your answer(s). Otherwise, your data will be saved and you will proceed to the requested screen. We also allow you to
skip to another screen without your new data being saved. Be aware that if you use the Skip & Jump function
on the left column, data on the current screen will not be saved and will have to be re-entered prior to submittal.
Once you have completed the application, click the 'SUBMIT' button located on the left column to submit the
application.
